Defining the Tunstall Integrated Care Platform

The Integrated Care Platform stands at the very centre of Tunstall Healthcare's Connected Health service. The Tunstall ICP is a modern software application which connects to a broad range of health monitoring devices via Bluetooth. The wearers/users of these devices, as well as their nurses or caregivers, can track their readings through the application for an accurate picture of their real-time health, and adjust their treatment accordingly.

Through ICP, Tunstall's line-up of healthcare devices and platforms enables its wearers to monitor their vital signs, including blood pressure and oxygen saturation. The application is also accessible via mobile device, ensuring you can keep track no matter where you are.

Clinicians can review your vital signs and contact both you and your GP/healthcare team should your data reflect abnormal readings.

Having access to real-time and historical health data doesn't just help you take control of your health; it also grants your caregiver, doctor, and healthcare team immediate access to your clinical data. It allows them to craft a tailor-fit solution that addresses your primary condition and improves your quality of life.

Since our ICP helps track your vitals from anywhere and in real-time, you'll be able to understand and take control of your healthcare. In the long run, this leads to immediate, at-home care and reduces the need for GP/in-hospital visits.

The remote tracking of your vitals also gives your caregivers and doctors access to your historical health data. With accurate data in hand, your healthcare team can rapidly diagnose and treat your condition, which prevents your health from worsening.

Partnering With Corumbene Care in Delivering at-Home Health Solutions

Corumbene Care is a local, community-based organisation that provides home and primary health support and services in the Derwent Valley, Central Highlands, and Southern Midlands regions of Tasmania.

The organisation works hand-in-hand with its clients to help them understand and manage their health, plan goals for their well-being, and provide access to in-home services. In doing so, Corumbene Care aims to reduce its clients' need for personal visits to the doctor, hospital or emergency department.

Corumbene Care partnered with us to better provide remote, end-to-end, and at-home services. Clients were given remote monitoring devices, such as personal alarms and fall detectors, connected to Tunstall's ICP. These devices allow wearers to take their vitals and send readings via Bluetooth to our ICP app.

Through the ICP app, Corumbene Care's nurses can remotely view their clients' health data and respond immediately to any abnormal readings. They can also pass this data directly to the client's healthcare team to facilitate the necessary care.

Dan's Story: Reduced Healthcare Costs and Quick Recovery

Dan, a 40-year-old small business owner, suffered from a blackout at work back in 2017. He was taken to the Royal Hobart Hospital (RHH) Emergency Department four times in two months and was put on the cardiology clinic’s waitlist for ten months.

While on the waitlist, Dan experienced memory loss, physical coordination problems that kept him from driving, and increased anxiety. These escalating symptoms pushed Dan to seek solutions from Corumbene Care.

The Solution

Corumbene Care worked with Dan to develop a customised care plan to help him:

●     Improve his knowledge of his condition,

●     Monitor his vital signs (such as his blood pressure, pulse rate, weight, and heart rhythms) with devices connected to Tunstall's ICP, and

Seek the services of a private cardiologist through GP referral.

Because of Corumbene Care and the ICP's active and integrated involvement in the healthcare plan, Dan's cardiologist and GP received his updated health data that enabled them to come up with a treatment plan and medication.

"They (Corumbene) rang and checked in on me in between medical appointments and when they received abnormal readings from the monitoring equipment," said Dan. "They prevented me from having to be admitted to hospital, which relieved another huge burden."

The Results

Dan's health stabilised after being with Corumbene Care for two months. He no longer needed hospital admissions, ambulance trips, or emergency department visits, reducing his healthcare costs by 60%.

"I believe the program relieved an enormous amount of stress because I could see and understand what was happening and also see improvements," he said. "I also knew that if I had any questions or worries, I only needed to call."

Not only did Corumbene Care Dan help Dan reduce his healthcare costs, but the program also helped him return to work, saving both his business and employees.

Conclusion

Tunstall's ICP partnership with at-home healthcare service providers like Corumbene Care is a powerful combination that empowers people to take their care into their own hands.

It involves them in their care, giving them an active role in managing their condition, and as in Dan's case can help them recover fast and get back to where they need to be.

The ICP's real-time monitoring and health data collection also allows for rapid diagnosis, which keeps health emergencies from worsening and reduces hospital visits. People can worry less about healthcare costs and focus their attention on what matters—their health and improved quality of life.
